There was an interesting incident on a Survey vessel off Oman, the vessel was equipped with an underwater position reference system part of which included a pole on the end of which is a tracking head, this was lowered by a chain hoist through a hole in the bottom of the ship, To stop water ingress when the pole was recessed into the hull. The tube with a gate valve was fitted a the hull bottom. When lowering the pole with the valve still shut the chain broke, the pole on its way to the sea bed took out the gate valve. The ship is now left with an 18 inch hole in the hull which the ocean is filling, the ship despite the actions of her crew followed the pole to the sea bed.
