I've been living in Japan for 5 years now and to be honest, I've heard of such restaurants, but I've never seen one before. There are some bars that are for members only, or you have to have an introduction to get in, but these are not only for foreigners, but also for Japanese. I've also seen signs saying "Only Japanese", but that doesn't mean that foreigners are banned, it just means that the staff here only speak Japanese. Anyway, I have never been refused because I am a foreigner and if such shops do exist, they will be few and far between.
