  1. I'll give it 6 months before it get slowly picked off with exceptions. An example is you go for an outpatient procedure, the doctor request a lab or X-ray, the lab is in the hospital and you're already prepped for the procedure, they perform the tests or take the X-ray, guess what, all involved are in network except for the lab IN THE HOSPITAL. Happens more than you think.
