I don't think this was a false flag operation, but either a complete breakdown of Russian counter-terrorism operations, or (from my perspective more likely) the FSB allowing the attack to proceed so they could use it for propaganda purposes. If the latter, I think what happened is that the terrorist attack got out of hand.
