That is not the standard actually. Military service is not needed for naming.

Most repairs are temporary. To stop fire and flooding and keep power on. Any hull damage requires drydocks or divers. So for something like armor, your just planking over the holes or welding sheet metal as a patch. You can not bring the strength back to 100% at sea.

FRAM was a life extension rebuild for Sumner and Gearing class DD's. The idea was to extend the life of WW2 ships and update them for modern ASW weapons. So various guns, torpedoes and depth charge racks were removed and replaced by new missiles, guns , ASW gear and fire controls.
