  2. That is no always the case.  I do tip when in Europe with notes but it depends on where I am eating and the actual cost of the meal.

There is no comparison.  Eating is cheaper in America unless you are going for Michelin-starred standard restaurants where there is generally no difference in cost.  The culture started in America in a big way when they where richer than the rest of the world when there standards of living was way beyond that of Europe.  Post WW2 Europe and that includes the UK was very poor the average person could not really afford to tip. This is the reality.
