The difference is Americans pay for their own health care, and a lot use "insurance", or paying a monthly fee and shoving the expenses back to the insurer. In Canada and Europe, you pay taxes, and then the government pays for medical care. The issue arises in that government is inherently inefficient, so costs go up or care goes down.
