The USN training film on the Forrestal fire is a great film that teaches a lot of lessons, it was still being shown at least as late as the early 1990s. Wouldn't doubt the Navy still uses it today. One thing to note about that fire is that, even though they made mistakes and didn't know exactly what they were doing, the crew showed great courage in continually running back to fight the fire and try to rescue their buddies, you can see this in the film: a bomb goes off on the flight deck and a bunch of guys run straight into the fire, huge balls.

Regarding discipline and punishment, read Saburo Sakai's book. He was a Japanese Navy fighter pilot and his description of pilot training is vivid. Japanese naval aviators were severely trained and beatings were common. The training regimen was very long and the washout rate was very high. They were trained not only as pilots but also in sword fighting and hand to hand combat, and those who made it through were basically considered as samurai. Great book, even if some of his stories are disputed.
