First, he got some training at his advanced age that would generally only be available to younger candidates, he was sent off to a post without supervision, he was obviously, with many reports about it, in over his head at the post and he ignored signals and warnings by the systems that he should know mean trouble. He essentially had every information necessary to avoid this disaster and didn't.
