There are people living in such way, even in western europe. It's mostly their own choice, and often do have a okay pension. Also in Norway some choose to live at places with no electrisity, and sometimes no road for car. But there a very few of them left these days. From what I have experienced it's mostly out of sentimental reasons, people lving in old farm houses where they were born, farming now ended long time ago, wanting to stay there as long as health allows. I knew one in Norway in the 90's. A outgoing older fellow, well known by most people in the county. Money no problem, good pension, but prefered to live in the old crumbling house he was born up in the mountains, no water, no electric. He felt the house was not worth putting any money into, just kept the roof sealed from the rain, and he was happy enough in his small kingdom, and old mountain farm (not operating)

there are some poorer countries in Eastern, south eastern europe,especially, where there are quite many like this man. He have chosen to live where he does, but it's not that far away from neighbours, town. He got a small pension, and some people, a social worker, he meet from time to time. I would say most get offered some sort of help/care, if they wish. What is offered might not always be that much, especially in some poorer areas
