Caliban777 It wasn't just the sub torpedoes. The torps on DDs and PT boats and those on the TBD had issues too. Then there was the 1.1" AA gun. It's almost a wonder the 5"/38 turned out well. As for bureaucracies, management at companies I worked for in the private sector were just as oblivious to the shortcomings of their products and business models. One company circled the drain until it was bought out, the other went BK and largely liquidated. If BuOrd had been as badly run as those companies, it would have persisted, insisting it's torps and the 1.1" failures were all the fault of the crews using them, and would never have adopted not-invented-here Bofors and Oerlikon guns, or effectively addressed the issues with the Mk 14.

Caliban777 All true. Then there were the contractors that Harry Truman had to investigate. B-26s with rookie pilots at the controls crashed with alarming frequency, because the wing loading was too high, but Martin, or the Air Corps, didn't care. Quality at the Wright engine plant near Cincinnati was horrible. There was quite a scandal about the Army QC inspectors being paid off to pass substandard engines, as Wright was more concerned with making more profit, then the lives of the men that manned the planes those engines powered. At least BuOrd saw the light and changed it's way of operating, so the deficient weapons were either replaced or modified.

Caliban777 High performance aircraft tend to have high wing loading because the wings are made short to reduce drag. High wing loading also tends to mean high take off and landing speed, where most accidents happen. Pappy Boyington is said to have really liked the F2A when it first came out, said he could turn it in a phone booth. But then the Navy wanted self-sealing tanks and armor. Weight and wing loading soared and the maneuverability went away.
