  1. On kamikaze: I wonder how much cultural factors make a difference to whether or not suicide attacks were seen as sane or not. In the (post)Christian West, suicide is taboo: not so in other cultures, including pre-Christian Rome. But even in Western forces, the notion of a "suicide mission" is recognisable (for instance, operations by Fairey Battle squadrons during the Fall of France), and posthumous decorations must often have been awarded to people who had no very lively hope of survival. But there is still a difference (I guess) between pressing on with suicidal bravery, and deliberately ramming.
