  1. I think that the main cause is America's focus on cars for transportation. Have a look over here in the Netherlands and the rest of Europe, where we have a completely different infrastructure. As a result, people have more access to efficient & affordable public transportation, can ride a bike on a safe way and can take a walk for some groceries in a nearby supermarket.
