Companies pay the least amount to get the functions they need out of their IT systems, many have in the past placed very little value on securing systems properly even though the solutions are already known. They pressure IT staff and contractors to get things done in a hurry and two areas almost always suffer security and documentation. The solution is to not trust people with any more data than is absolutely necessary and demand assurances that they meet world's best practice for data protection. Also keep this in mind, there has never been a time when Microsoft systems were secure, if you look at the timeline for all of the discovered vulnerabilities and when they were first active you discovered that there is a continuous overlap from one vulnerability to the other. And that is without throwing "stupid humans" into the risk matrix.
