  1. Getting fuel to the engines in 0 g: there are a few methods. One is to do an ullage burn: run some small thrusters to provide enough acceleration to settle the propellants at the bottom of the tanks. This is usually done for large engines. Another is to have fuel in a bladder under tension, or to have a structure in the tank that the fuel will tend to adhere to, which can wick the fuel into the engines. These can be used for small thrusters.

  2. They never reached orbit. The Shuttle used its OMS to add the last few m/s of speed to get into orbit.

  3. Ping pong? I've never heard that term. There is a phenomenon called pogo oscillation, i.e. a back-and-forth movement of the fuel on the longitudinal axis of the rocket.
