The first time, was more likely than not, an accidental release, by rebels mishandling something shouldn't be messing with. The second time, could've been released by an airstrike hitting a rebel stockpile, or as an unapproved operation. You have to consider the conditions and state of the chain of command, at the time. At any rate, neither incident was characteristic of a tactical deployment of an inventory weapon.
