Simplest answer is 'limp-wristing' can cause a doublefire, with a semiautomatic handgun. Trigger is pulled, gun fires, recoil rocks the gun in the hand and clenching to hold on, fires it again. It can happen so fast, it seems like a single shot. The reason the TV/Hollywood hostage rescue headshot, isnt actually a thing, is because unless a target the size of a walnut, buried in the center of the head is squarely hit, the central nervous system jolts and causes the hands to clench(origin of the term 'death grip'). Its a common condition to find suicides clenching a gun, in a firing grip, finger on the trigger; and there have been a few cases of medics and coroners accidentally discharging the gun, trying to remove it from the victim's hand. You can even lookup some 'gun fail' videos and see people doublefire a revolver from limp-wristing. Yes, its entirely possible to shoot yourself in the head twice, with a semiauto handgun.
