Chances are, there was an assumption. Everyone's paperwork was in order and valid, so because this is all on the up and up, they undoubtedly assume there's no reason why base and other branche commanders wouldn't know what NASA was doing.
We hear about this stuff happening all the time. It's literally what everyone has said: left hand not knowing what the right is doing.
It's nothing shady, it's just assumptions based on sound logic. Problem is, no one likely bothers to look at the paperwork, or look FOR paperwork explaining things. (further likely that they do not poses a cross-agency search function like we have with our home computer or the internet)
Going forward, it would sure be prudent to mandate that when filling the paperwork, a phonecall must be placed to the major government parties.
Will it solve things? Likely not. But at least once this happens next time and it starts making its rounds in the news, a handful of people would say "oooh yea, that thing I got a call about, I can clear this up!" 😊
