I have witnessed the crowded campgrounds, the crowded beaches, the license plates from all over, the crowds of people tight together and ignoring masks & distancing unless some plucky store/restaurant employee forcefully reminds them of the rules. With the current high prevalence of virus and random community spread, there's almost no chance those people aren't circulating the virus.

Here in Oregon (pacific northwest USA) we've just learned that an elementary public school employee, in a suburb of Portland, has been symptomatic since February 19th and has just tested positive for covid-19. Most likely, they contracted it on or around February 12th (from sources unknown) and have been communicative since at least February 14th. This is time enough for it to spread to the young students (who will make excellent carriers, as they suffer it fairly lightly compared to older adults with medical issues) and their families and whoever those people have collectively been in contact with. If any of those people work in public-facing jobs (as many people do) then Portland is effectively already undergoing early stages of pandemic. 95% of the human population in Oregon (around 4 million total) lives in the Willamette valley and the intersection of the Columbia and Willamette rivers. Vast flows of daily commuter traffic go in and out of Portland via routes 5, 205, 30, 26, 84. Looks like we're going to get a firsthand look at how things unfold here in the USA.

The US state of Minnesota has just opened industrial and office work settings .. but virus cases in Minnesota are rising exponentially, and 7 of the past 9 days have seen consecutive records for the most new cases per day. Offices in particular will be virus incubators. I tire of reminding people that you can recover from being broke, but you can't recover from being dead from the virus. I'm on 48 days without any kind of income, and relying on friends to help me stay afloat.

Yet this morning at a supermarket, I kept running into the same woman who wasn't wearing a mask, and who ignored all the evil eyes she was getting. Domestic travel in the US still continues unabated, people still gather on beaches in Florida, people still go to spring break in the sun, some states are not yet shut down, etc. Even if most of the country is locked down, those people still socializing and gathering and traveling will spread the disease throughout the country. Air travel, buses, trains, boats, automobiles still continue to go everywhere. A little over 0.3% of the population has been tested.

What was the one before that? These things play on false hopes, and when clinical trials are done, the results almost always end up being at best inconclusive. Randomized double-blind trials are the only thing that prove it one way or the other, and so far the various flavors of snake oil are not doing well.

Young, old, healthy, sick, it does not care who you are, it can kill you with ease of there is not hospital care available for you when you need it. And that has always been the danger of this disease, overwhelming the local hospital systems. When that happens, the death rate goes up from around 2% to around 20%.

But then, those countries don't let a predatory monopoly set prices for things. In some places, you can even set up a drive-through test for free. It was revealed yesterday that the time of greatest communicability is in the first 3-5 days of incubation, which is well before any signs start to show. Those mushrooming positive-test numbers in S Korea are lagging perhaps 10 days or more behind the actual spread of the disease - that is, if you could freeze this moment in time and methodically test everyone in the country. Many are not showing symptoms yet but are in fact infected already, they just don't know it yet and would test negative for it at the current moment.

  208. Here in the US (and probably everywhere else) the level of effort needed to pull this off is much underestimated. Even if you had free walk-in vaccination clinics at every pharmacy, every supermarket, every library, every town hall, and they were open 18 hours every day, it would still take quite awhile to vaccinate everyone.

  211. It is actually a lot worse than just that. Of the 40 states that have lockdowns, some still permit church services. Local and interstate passenger traffic (all forms) still continues. The CFR gets decoupled from the penetration rates when hospital resources become overwhelmed, which is what will happen everywhere in the US soon. NYC is actually a best-case scenario for the US due to its relatively high quality of healthcare. Louisiana, Florida, Michigan will show what it's going to be like in most parts of the USA :(

An astonishing 13 states (AK, AZ, CA, CO, HI, IA, ME, MI, MS, OH, RI, SC, WA) were missing their testing data for yesterday. Some states even showed a "sudden" new-cases-drop right at the same time they posted a huge drop in the number of tests performed, go figure.

We know already that people start spreading the virus as soon as 2 days after being initially exposed, and that up to several weeks can pass between initial exposure and testing positive. So if that one student has tested positive, chances are high that it has already been passed on to the families and anyone they've been close to. Do any of those high schoolers work in restaurants? Extend that to everyone who's been to those restaurants and their families as well. That's how widespread it is by now.

We need to move slowly and carefully on this.

What was different with the recent protests at the state capitol is that people clashed with the police (breaking into the building) while wearing body armor, pistols, and carrying long guns and chemical weapons. That's not normal for here.

Only a limited number of people allowed in the store at the same time. Shopping carts were being sanitized between uses. Distancing was being enforced in lines and at the checkouts. It seems the delay between a large event and the spike in related cases is around 42 days +/- 4 days.   404. @cowsmart1 This thing is already out of the bag. The one known patient has been feeling unwell for at least a week so he's stayed at the hospital. That means he's been infectious for at least 2-3 weeks prior to that (depending on how fast/slow the onset of symptoms was) and that everyone who's stayed at the hotel or encountered him in that time is now a new potential vector that will crop up in a few weeks after they get home and start making new clusters.

  417. Well, for starters, states like Missouri supposedly had huge numbers of people previously-exposed to covid, yet that's one of the hardest-hit states for Delta. Missouri's overall infection rate is around 105 per thousand. Figure 10x that many were infected and had it asymptomatically. So if prior exposure led to immunity, the rate of new infections would be low, but instead it's pretty high. Same thing in Florida at 114 per thousand, yet a very high rate in new positive tests.

For every normal pre-pandemic human activity, you can derive a "how easily could the virus spread from person to person" factor that's dependent on the following variables: - Is it indoors? - Are there people from different households there? - How good is the air circulation? - How close do people get to each other? - How long are random people in the close presence of each other? Supermarkets, and other large stores that can afford to upgrade their air circulation, are doing mostly OK in terms of not spreading the virus from customer to customer. The same unfortunately cannot be said for the workers in those places, or for smaller businesses that can't afford to be compliant with these measures. If all you're going to do is whine about your "freedumbs" being infringed upon, then it's a good thing you weren't around for WW2, because people got asked to do quite a lot more in order to save fewer lives than covid will cost. The authorities have had to make some hard decisions about what constitutes an acceptable loss on many fronts. Unless we're prepared to deliver food to everyone's homes (as China did during the worst of their lockdown back in Jan/Feb/Mar/Apr) we must keep those stores open. But everything else should probably stay closed. The alternative is going to look like 3000, then 5000 or more deaths per day.

  445. What I've done is download the data to a spreadsheet (it's available on either state or county level, with daily timescale) and put those two on the same graph. Deaths generally trail a month behind cases, when you're trying to see the relation between the two. It's a little muddled in places because the death rate goes higher when local healthcare resources are overwhelmed and someone must make the choice of who to save vs who to send home with aspirin.

For example, in a country where no advanced medical care is available, mortality rates could be as high as 15%+ due to lack of oxygen/ventilator treatment. Italy has already used up all of its capacity to do oxygen support. Many societies can not afford to lose 10%-15% of their population, with many of them being experienced senior people in positions of authority.

Michigan alone had 4500+ new cases that day, NY state more than 12K. I was refreshing the page and suddenly the column flipped over to happytalk-style numbers, although if you saved them in spreadsheets and subtracted one day from the next you could get the true picture. Where I live (Oregon) has nearly the worst testing rate in the whole country (only Idaho tests less per capita) so with the state unlocking I am not confident about what's happening next.

When it is operating in a confined environment (such as aboard the quarantined cruise ships) it produces a higher percentage of asymptomatic carriers who, once they escape into the general population, suddenly go from testing negative to testing positive, but only after they've infected other new hosts. It hides so well in apparently healthy people. From a news report yesterday we learned that the greatest virus shedding occurs in the first 3-5 days of incubation, which is before symptoms even begin to show. SARS was much slower-moving by comparison.

Because in this sorry corporations-are-first-citizens place, not only do you get charged for the test but you get charged for your quarantine later. Poor people will not be tested. Ordinary people were visiting the disease hotspot in Kirkland, WA then flying home by regular passenger jet. People flew back from Italy and resumed their normal ways. Same for people returning from cruise ships. And looming in the background is this strange unwillingness (or inability) to do the widespread screening that would help reveal how widespread it really is. Community transmission has been happening in the US since at least early January, if not even earlier.

From the numbers, it looks pretty clear that naturally-acquired immunities aren't anywhere near as good as what the mRNA vaccines provide.

From their videos I can see that lots of people there have not been observing precautions all along - business as usual with restaurants, stores, social gatherings, not a mask to be seen. Where I live (the county is 67.2% fully vaccinated for 16yo+ people, 54K covid cases per million overall) the Indian variant is here already but not spreading very quickly. Mostly it will affect the people who are vaccine averse and who don't observe precautions - usually they are the same people anyway. I am closely following the numbers for counties where vaccination rates are lower, and from what I'm seeing, the protection from prior community exposure is not as good as the protection from vaccines. For example, we've got a county where 48.8% of the 16yo+ population is fully vaccinated, where over 10% of the population has at one point contracted covid, yet the new-cases number has been going up steadily for 8 weeks in a row. I fear that the double whammy of a) low prior community exposure and b) new variants on scene will cause bad outbreaks in places that until now had been doing fairly well.

Yet this morning at a supermarket, I kept running into the same woman who wasn't wearing a mask, and who ignored all the evil eyes she was getting. Finally I looked at her pointedly and tapped the mask. She shouted out that some people are exempt (they're not) and that some people are smart. Huh. "Smart" like Herman Cain, she obviously meant. I moved away from her quickly after that. At least she didn't shoot me.

  715. Dr John et al .. last night I ran the new cases per M per week for the whole USA down to county level. It would appear that the new variant may already be in at least 15 states in the USA. San Diego county already appears to be undergoing its new-variant runup, but at roughly 2-3 weeks behind the LA curve. The California counties are the hardest hit at the current time of course. Data source was the NYT csv file. Edit: that new variant appears to have been here at least since around week 45.

  729. Reporting from Portland, Oregon, USA. Last night I went past at least 8 restaurants that were completely packed with the 20-50+ yo crowd. Not a mask in sight. This was kind of strange, since Oregon did not open vaccination to the general adult population until 4/19. Even if the entire eligible group had gotten vaccinated then (they didn't, as we know from the numbers) their second shots wouldn't be until around 5/19, and their full immunities would not be until 6/2. Community spread of the B.1.617.2 (Indian) variant has already been found in the Portland metro area. Some stores in Portland are letting people come in without masks based only on the honor system, without actually checking to see if they've been vaccinated, or simply taking people at their word when asked. This will not end well :(

Assuming covid-19 runs loose in the USA (as all indications point to), and 80% of the population is infected within the next 4 months, here's what we're looking at in terms of effects: 327,200,000 people 261,760,000 affected (80% of the number above) 52,352,000 sick enough to require hospitalization (20% of the number above) .. and this is where the poop hits the propellor, becaue clearly we haven't got the means to hospitalize that many people and/or perform oxygen support for them. In fact there are less than a million hospital beds in all of the USA. So of those 52 million severely affected people, many will die, and many more will be out of the workforce for an extended time.

On the way home from some solo excursion to the woods the other day (no people around, no risk of virus, right) I saw a few different crowds of teenagers pressed close together playing basketball and other games and milling in crowds. Unbelievable. Then again, there hasn't been any kind of pervasive, all-channels-media effort to blanket the airwaves with constant PSA's telling people to keep apart from each other :(
