If OP's account is true then he would have fired in a different way. Had he believed an armed intruder was behind the door he would have spread the grouping and emptied the magazine. This is because he would have imagined the intruder was standing and would return fire. A logical explanation for the four shots aimed around the same (lock) area would be that OP imagined RS to be sat on the toilet (perhaps where she had just been talking from) when he lost it and decided to assert control by firing his gun. Unbeknownst to OP, RS had stood up to open the door and was quietly touching the key wondering if she should unlock the door. The only other possible explanation is that OP could hear and therefore knew RS was behind the door and blasted her in anger........ Or OP believed he was being burgled by a dwarf.
