  1. In the B-52 crash - what little I know about it - the pilot in command had a history of 'reckless' flying, often ignoring safety regulations. He was never officially reprimanded as he should have been, and eventually his 'shenanigans' caught up with him AND THE OTHER THREE PEOPLE on the "BUFF" he was flying. Unfortunately.
