The closest thing to a "failed sea trials" I can think of is the German frigate Baden-Württemberg in 2017. For the first time in German naval history, an allegedly-completed warship was deemed completely unfit for service and sent back to be rebuilt.

@panzermensch1577 Primarily because the triple 28cm turrets were available right away (they're a carryover from the original plan, in which Scharnhorst and Gneisenau would've been the 2 D-class panzerschiff, plus being a pretty simple upgraded version of the Deutschland-class turrets), whereas the 38cm twin turret hadn't actually been designed yet. When the 20,000 ton D-class cruisers were cancelled in favor of 32,000 ton battleships, there was serious consideration almost from the very start to switching to 38cm guns, but ultimately Hitler decided to stick with the 28cm so he could have his battleships as soon as possible.

@TheFreaker86 AA mounts tend to have unlimited 360° rotation, as do a lot of post-WW2 autoloading guns. As for larger guns, the German 15cm/60 triple turrets on their light cruisers could rotate a total of 720° (2 full turns).
