Is Admiral Callaghan the only case of a Medal of Honor being awarded for the specific action in which the recipient personally killed another Medal of Honor recipient via a friendly fire incident? BTW, we do know that the US doesn't automatically award the Medal of Honor to admirals who die in action, because Rear Admiral Theodore Chandler only got the Navy Cross after dying to a kamikaze attack on his flagship. This despite the fact that Chandler showed actual heroism, personally aiding in the damage control efforts even as he was dying from his wounds.

@amir-ng6jv Torpedoes are projectiles, so no. That 38cm shell would not be the single deadliest non-atomic projectile. The deadliest event that can be unambiguously attributed to a single projectile would be the Soviet hospital ship Armenia, sunk on November 7, 1941 by a single air-dropped torpedo, killing between 5,000 and 7,000 people. The German transports MV Wilhelm Gustloff and MV Goya had even higher loss of life, but they were sunk by 3 and 2 torpedoes, respectively.
