  1. Couldn't they have modified it to fire a larger grenade? While increasing the grenade diameter would've required redesigning the whole launcher assembly so that it's further offset to the side of the rifle, the other method that would've been possible is to increase to length of the grenade and the cup. Since such a change would've required no further modification at all to the rifle beyond just attaching a longer cup, it seems like it would've been something worth trying.
