Free at the point of use, it is still being paid for by your American tax dollars, it is very similar to how the way we deal with our (The NHS UK) universal healthcare in the UK, it is free at the point of use for everyone in the UK (but....we pay for our healthcare through national insurance contributions (National Insurance Contributions (NIC) are taxes paid by British employees and employers to fund government benefits programs, including state pensions.) and taxes here in the UK.
