To start, those individuals are poor and as Prof. Katherine Baicker put it, bad health is associated with those in poverty. So the question become do they die because they did not have access or do they die because they are in bad health to begin with? To extend, there are higher rates of smoking, obesity and type II diabetes with the poor, all self inflicted. And as mentioned in the book 'Being Mortal", people seek out modern medicine to live another 5 or 10 years but really live another 5 or 10 months. So if those people receive care but die in 5 months, is that a success?
