"You know what helps prevent a lot of people from becoming obese, being addicted to smoking, getting STDs???" How about 1. Eat healthy food and exercise 2. Not smoke 3. Not have unsafe sex practices and do drugs All things taught in school. I see a doctor once a year for a mandatory check up and I get STD check ups twice a year. That's it. I am physical fit, I don't smoke, and I do not have any STDs. No doctor needed to tell me what it took to get that way, it took my elementary school teacher that.

It is a very fair question. If someone else is going to pay for their healthcare where is the motivation to stay healthy? Obesity and type II diabetes rates are higher in poor community.

The logic is this, obesity and type II diabetes are higher amongst poor people. They have the simple option of eating healthy and exercising however they choose not to. It is a fair point.

Eh, not really. Two professors wrote a book on that subject. https://www.aei.org/wp-content/uploads/2014/03/-the-business-of-health_110115929760.pdf The issue is more complicated than what you think. There is also the book "Debunking Utopia" that gives a good breakdown of that as well. To simply say "look at nordic countries" either you being lazy or you being foolish as people have looked at those countries and will disagree with you.
