Several reasons beyond healthcare such as eating habits, health habits, etc. If you remove car accidents and murder, two things not strongly connected to healthcare, than the US has the longest life expectancy. Now that is not to say that the US is number 1. What that means is that the difference in outcome, like life expectancy, is so minute that you can't use that data to say if one country has better outcome.
A few years ago I had the average life expectancy of the world to be 71 years with a standard deviation of 7 years. The US was just over 78 years, so one standard deviation over. The highest was 89 years. Other countries like Norway, France, etc, were only a few years over the US. That is not much when you factor in the many variables involved.
In short, you can't just use pure life expectancy as many factors contribute to overall life expectancy. You are saying that life expectancy is only dependent on healthcare meaning if I were to get hit by a bus today that is only because I had bad healthcare. That is what you are saying.
