  7. Most accidents these days are not single point failures. Aircraft for instance. Everything is designed with failure in mind. More engineering time is spent on failure (real AND imagined) than on the initial design in high rel equipment. Double redundancy if failure is inconvenient. Triple/quad redundancy if failure is catastrophic. Safety margins of 10% if weight is an issue.
