@weekiely1233 Most sub accidents are human error because bad designs were rejected during certification. Just like the bombers that didn't come back were shot in critical areas, the subs that would fail at sea failed the certification process.

@weekiely1233 Most are human error because we minimized hardware faults first. Neglecting the hardware because human error is the main cause is how you start having non-human caused accidents because the hardware was faulty. So yes, you're looking at a selected pool of data and ignoring the data that was culled. Choice of materials or poor assembly is also a hardware error. A sub pilot dying because his sub was poorly constructed isn't a sub pilot error. A sub pilot crashing a perfectly good sub into the Titanic and damaging it is.
