Hate to break it to you, but actually, it's not that hard algorithmically speaking. I wonder, why they didn't do it before. You see, every city contains the same objects, that may vary in forms and style depending on the type of the city (megapolis, urban, ancient etc..), but in general it is buildings, roads, bridges, traffic lights and stuff like that. So first, you create those objects, then you multiply them by copying and then, you write a function that would place those objects on the terrain in some preconfigured order according to your defined rules. The concept is pretty simple, but to perfect the realization you must work on it quite a bit. Just, it's not a big deal though, if you have a huge company.

@stucclikechucc That's right, it's all about the money. They're trying to create a new Market so they can sell more stuff to more people. Now, in 2021, they're building "worlds" for that. If you own a world, you own it's residents too. You can make the rules, you can make changes, you can decide whatever you want, you have the "lives" of the residents with their money streaming into your pockets... If they're living in your reality, you can do whatever the hell you want as long as they want to stay and play. That's all this "Metaverse" or "Virtual reality" idea is all about.
Now think, who are going to the players? Most likely age group from 12 to 35. That means, young people, because the mature and old don't give a damn about all this "matrix" shit, no matter how good the graphics is. Imagine a whole generation that lives in fiction world and doesn't know the what's real. They probably wouldn't even have children. They will be too lost in some fantasies, that not even theirs.
