  1. Here in Finland the solution is to have no fuse in the extension cord but have extension cords beefy enough to not be the limiting factor. In practice even the cheapest extension cords here can carry 16 A which is usually the maximum fuse used in normal homes when we have 230 V system. For US I'd think easy solution would be to require minimum wiring for 20 A or require built-in fuse if lesser wire is used. Considering the price of copper, most manufacturers would probably opt-in to built-in fuse.
