  3. Karen is not used as a name but an adjective and we can distinguish between being Karen and named Karen. The problem is the english language is not very good in creating new words to describe things fittingly so this probably goes back to the first case who happend to be named Karen. Other languages can combine words to make a new term in cases like that.
