  2. Yes Jihad can mean a spiritual self searching but anybody who is not being naive can guess the context in the majority of situations.

  3. Do we know where he qualified to be a doctor and has anyone bothered to check. If he is happy to have a different name he could just as well choose to have a different occupation. I am just saying it is in the realm of possibility and would not be the first time. As for hatchet jobs the only one I see is the one being played on the British public.
