  1. I've heard of Mayak. It happened the same year as the Windscale nuclear reactor fire in the UK which was called "Cockcroft's Folly" before the fire, because the project leader John Cockcroft fought to have filters installed at great expense and delay time. The filters caught 95% of the radioactive dust when the reactor caught fire, preventing Northern England from turning into an Exclusion Zone like Chernobyl. I wonder if the project leader at Mayak pushed for safety too but was mocked and ignored.
