For me in the UK, I can travel around a mile on foot without any problem and it only takes around 25 mins, beyond that, I take a bike, bus or car, and for me, it's normal and I don't think anything of it.
As for meals, I've never had any issues with meal sizes, for me at least, the portion size are about right, but I do remember my brother saying when he went to America that the portion size are much bigger, and that was in 1991 when there was far less of an overweight issue in the US, but clearly, it's portion size and not doing enough exercise that's likely leading to the overweight problem in the US, whereas we Europeans probably don't realise it, but the amount of walking a lot of us do, it's exercise in its self, which probably helps to having a healthy lifestyle, especially when the quality of food in Europe tends to be higher quality in the US.
I think it also helps in Europe that it's a much nicer place when it comes to walking, so much so that I don't feel the need to rush, whereas the US isn't very walk friendly apart from major cities.
