  1. I don't see what could be wrong. If you accept "Lead used to be added to Gasoline" then I don't understand why you would reject "Why lead used to be added to gasoline" It is not a complete sentence, but a title need not be.

  3. That date for outlawing leaded gasoline as an automobile fuel in the US in the '60's cannot be right. I owned a '70s era Imperial which ran on leaded and I could still buy leaded to put in it at the gas station in the early to mid '80s.
