It's clearly the armorer's fault. No actor would risk his or her reputation by intentionally shooting a cameraperson and bring that much negative attention and PR to him/herself. Many will insist that he's guilty, but the AD (Dave Halls) admitted to yelling "Cold gun!", and that's all there is to it.
