The brakes being used were out of date technology, and upgrading the brakes would have alerted operators to the high heat. In addition striking workers have called out 'precision scheduled railroading' for its lack of time given to safety checks. These are both relatively basic answers that you should know if you've spent any amount of time learning about the Ohio disaster. In addition, it doesn't take much to find out that they spent $10 billion dollars last April to buy back their stock instead of investing in their business.... Which is massive corporate negligence.

@bluewater454 Yes, the problem was the braking system that didn't have the ability to inform operators as well as modern systems, not the brake specifically at the point of frictional contact. in addition, maintenance of sensors was glossed over due to precision scheduled railroading....
