  1. To be fair, the Mk 14 was a very good weapon once they got the bugs worked out; it's more a case of failure to beta-test rather than one of an inherently bad product.

Even with most of a century of additional engineering knowledge, getting pure oxygen to start burning fuel without explosions happening is still quite tricky today (nowadays, that particular problem is seen mostly with development of rocket engines). I suspect the early attempts may've been suffering from what's known to rocket engineers as a "hard start"; when dealing with potent oxidizers, like pure oxygen, the reaction has to start immediately upon the fuel and oxidizer coming into contact, or else you initially get a slug of fuel and oxidizer accumulating in the combustion chamber, and then it ignites and BOOM.
