It is not possible to overemphasize the necessity of keeping sufficient water in the boiler at all times. Boiler explosions have also occurred when a sight glass is clogged, giving a false reading leading the engineer to think there's more water in the boiler than there is.

My sympathies to the crew. What a terrible way to go. Undoubtedly it was a crown sheet failure, releasing the pressurized steam in the builder. Often times when this happens the boiler will blow right off the locomotive chassis and catapult through the air like a twig. The force is unimaginable.

@liboud22 It does sound far fetched and who cares anyway? The main message is to be extremely careful with steam locomotives, or any pressure vessel for that matter, and know what you're doing. I've heard of crown sheet failures due to a clogged sight glass giving an incorrect water level reading.
