  3. Worked for the largest bank in the US that is always in the news. It gets so bad that asking is a monumental task so to have a peaceful life it was easier to avoid asking for change. Let's not even talk about the unaccountable teams that make changes that affect all their customers without any consultation at all. When things blow up it's impossible to find who is responsible.
