  1. That's a great comment. I'm an engineer who works in industrial control systems and automation. That includes a lot of safety work because it includes things like emergency stops & emergency shutdown systems, safety gates, alarms, fire protection systems and all sorts of other stuff. I have even done some work on FPSOs, but nothing with trains. Its amazing how many times each year how close people come to catastrophe. What keeps us safe are the back up systems. When Sal got to that point about fires it was a massive face palm for me. Back in 2008/09 I worked on both the Varanus Island fire recovery project and also the Ngujima-Yin FPSO incident. One of the major issues with the Ngujima-Yin FPSO was the failure of the onboard fire system to work properly when the fire broke out from the gas compressor failure. It sounds like people are NOT learning from these incidents.

  2.  @taiwanluthiers  No cars come in fourth behind: - moto-cycle riders in third place who have to ride in among maniacs in cars; and - those amazingly brave guys who ride crocodiles across piranha infested rivers in second place; and - the pilots in first place of the Yokosuka MXY-7 Ohka rocket powered Kamikaze flying bomb. That thing was 100% fatal.
