@USALibertarian I think this is correct. Obviously, in such cases, mental health professionals should seek guidance. In general, when someone plans to do something illegal, it should be reported, while anything they have done in the past that they talk about is generally confidential. The difference between fantasizing and planning is probably an issue, though. Fantasies that patients report are usually strictly confidential.
