Professionals have every right to express concern because they observe behavior consistent with a serious disorder that places many others in danger. If they believe they have a duty to warn, I think they have every right to act on that. What they do not have a right to do is actually diagnose him, or anyone else who isn't their patient. Dr. Grande says that crossing that line has no benefit, since the 25th Amendment needs to be invoked, and everyone who can invoke it knows about the difference between concern about mental fitness and an official diagnosis, and knows that only treating mental health professionals can diagnose their patients. Trump can be unfit even without the diagnosis (or fit even with it -- harder to pull off, but possible).
