My personal GUESS is an irresponsible Peace Officer/SWAT Team Member was bored and had someone in their "Crosshairs or Iron Sight's" and they didn't have the damn Safety On like they should've and of course that's all it took for all hell to break loose. Would still likely be Manslaughter on said Peace Officer's part.
