The US Airforce (and military as a whole) has an offensive doctrine. They believe that the best defense is a good offense. That means, the US Airforce operates under the idea of air superiority, because they can afford it. Your not gonna see the AC-130 operate in hostile air space.

Well, the US Airforce is built for air-superiority. They would have all those threats dealt with before they send in something like the AC-130 or the A-10

I think the reason most countries don't adopt good ground support is because most countries are poor and have a defensive doctrine. Most countries see the Airforce as expensive and the least important. They'd rather protect airspace with AA than invest in expensive planes and the expensive pilots that have to man those planes. The US has an offensive doctrine and a huge budget. Ground support aircraft can't be sent in until the local air-space is secure. That means an effective and expensive airforce that is capable of air superiority. The US Airforce is the largest airforce on the planet. It can afford the loses it takes to secure a piece of airspace. Most other countries would rather rely on a few multipurpose fighters that if lost, isn't that expensive and they figure they can always rely on AA instead.
