Dyatlov and his two bosses (Bryukhanov and Fomin) were sent to prison. The government placed most of the blame on the deceased operators, Akimov and Toptunov.

What's true is that they broke numerous safety regulations that were intended to make sure the reactor never got into such a precarious state. Under anything remotely approaching safe and proper conditions the AZ-5 would have caused a minor power spike, not a 10000x spike and resulting explosion. As soon as the power dropped so low they were supposed to, and should have, shut it down for 24 hours to let the xenon decay. But there was political pressure to get the test done. And yes, insane boss Dyatlov is highly entertaining even if historically inaccurate.

The reactor should have been shut down the moment the power got so low.
