Drug and get it approved, and the R and D cost is figured into the price. There are several problems with that, the pharma company has to recover that money while their patient is valid, a relatively short time; many countries will only allow the drug to be sold at the price of current production leaving the company only able to recover R and D costs from US citizens, an example of this is Canada; finally the US Government makes it advantageous for the drug companies to provide these developed drugs at cost or even free to the third world. What this means is that billions in R and D costs can only be recovered from US citizens while we in effect subsidize drug development for the world. Blaming pharmaceutical greed alone absolves the US Government from making sure that others don't benefit for free from research done here.
