  1. My trust in providing access to information to government organizations evaporated many years ago. Following a fatality at work, I voluntarily provided as much information regarding our processes to the government workplace health and safety department in the interests of safety improvement. The department subsequently prosecuted our organisation (not me personally) but used every piece of information I had provided in an attempt to make me culpable for the incident. I was cross examined for six and a half hours even though I was not actually directly involved in the accident. Predictably, (simply because there was a fatality), our organisation was found guilty of sub-standard supervisory processes but with no convictions.
